The
Harrison County Hospital Foundation's Annual Sports Physicals will
be held Tuesday, July 7, 2008 from 5:30 PM until 8:30 PM in the
Outpatient Surgery Department of the new Harrison County Hospital.
Local
primary care physicians donate their time to offer free sports
physicals for junior and senior high school students planning to
participate in sports during the upcoming season at Harrison and
Crawford County Schools.
The
Harrison County Health Department will also be on hand to administer
free Tetanus, Diptheria, Pertussis (whooping cough) and Menactra
(meningitis vaccine). Consent forms must be signed by parent
or guardian to receive the vaccine. Forms are available at
your participating school and at the Harrison County Health

We
will begin taking appointments on June 16, 2009. For an
appointment, call 812-738-8708 on Monday June 16 during the hours of
12:30 PM to 4:30 PM; appointments are on a first-call-first-served basis.
Athletes
must also bring their IHSAA Pre-Participation Physical Evaluation
Form with parent and student sections completed.
