Harrison County Hospital Foundation FREE Annual Sports Physicals to be held May 18, 2010   

 

Free sports physicals will be held Tuesday, May 18, 2010 from 5:30pm to 8:30pm at Harrison County Hospital.  Local primary care physicians donate their time to offer free sports physicals for junior and senior high school students planning to participate in sports next season at Harrison and Crawford County Schools.

 

For an appointment, call 738-7869 on Tuesday, April 27 during the hours of 12:30pm - 4:30pm; appointments are on a first-call-first-served basis.  If you do not get an answer, please continue to call as messages will not be taken.

 

Athletes must bring their IHSSA Pre-Participation Physical Evaluation Form with parent and student sections completed.  Click the link in the box below for the form.  Forms are also  available at your participating school and the Harrison County Health Department.

The Harrison County Health Department will also be on hand to administer free Tetanus, Diptheria, Pertussis (whooping cough) and Menactra (meningitis vaccine).  Consent forms (click the link in the box below) must be signed by parent to receive the vaccine.
